When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
Please print off a registration form (GMS1) as well as the new patient questionnaire form. Could you please ensure that both forms are completed. Without these we are unable to register you successfully. Fill them out and bring with you on your first visit to the practice.

Please note your online registration request will be pending until you receive a response from the Practice to confirm your registration. We aim to respond within 5 working days of your request being sent through.
