Patients can now benefit from two services aimed at helping you help yourself with a range of structured support – they are our Social Prescribers (Live Well Wakefield) and our Health and Wellbeing Coaches. Both teams are here to offer information advice and support. Your social prescribing link worker would help to signpost you to a range of relevant services to help you with things like weight management, social isolation and debt support. These are most often non-medical issues that you are facing and you could well be facing more than one at once.

Suitable referrals to the Health and Well-being Coaches include:-
Type Two Diabetes control
Low mood, social isolation and anxiety, but not high level mental health conditions
You might decide that signposting with some short term help, is all you need. If so, to refer yourself to Live Well Wakefield, complete their online form https://www.livewellwakefield.nhs.uk/referrals/new , call them on 07881595448 or message them on Facebook @LiveWellWakefield. Please mention you have been referred by your GP.
On the other hand you might decide that a Health and Wellbeing Coach would be able to offer the additional advice and treatment you need. If so please talk to Live Well Wakefield or direct to your GP about a referral to this service.
