Travel Health Questionnaire
To help us offer the appropriate advice, we kindly ask all patients who are travelling overseas and require travel vaccinations to fill out a travel questionnaire. This can be found below
Once you have completed the questionnaire please return it to us and ensure you book a telephone appointment with one of our nurses to discuss your vaccination requirements. If a vaccination appointment is then required, a nurse will book this with you in the telephone consultation.
It is important to make this initial appointment as early as possible – at least 6 weeks before you travel – as a second appointment may be required with some vaccines. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

The Practice only offer the travel vaccinations that are included in the services provided by the NHS. For other vaccines, please contact the nearest private travel vaccination centre.
