Travel Health Questionnaire
To help us offer the appropriate advice, please fill out the online form before coming to see the nurse.
Once you have completed the questionnaire please return it to us. After 3-4 working days the nurse will telephone you to advise you on what vaccines are necessary and if needed, book an appointment for you with the practice nurse to discuss your travel arrangements.

It is important to make this initial appointment as early as possible – at least 6 weeks before you travel – as a second appointment may be required with some vaccines. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
The Practice only offer the travel vaccinations that are included in the services provided by the NHS. For other vaccines, please contact the nearest private travel vaccination centre.
